Requesting A Form From the SC Department Of Revenue

There are 2 ways to request a form from the SC Department of Revenue: by e-mail or telephone.  Read below for details on each method.

NOTE: When requesting Sales or Withholding forms, be sure to include your S.C. ID Number

Email:
Send the request to Forms@sctax.org.  Please include the following information:

• Name and mailing address where you like the form(s) to be sent (DO NOT include personal information such as social security numbers, file numbers, etc.)
• Form ID number, and title of form (for example, SC 1040 - Individual Income Tax Return)

NOTE: Form orders over 2 pounds will be sent via United Parcel Service (UPS).  You will be responsible for the Cost of Delivery (COD) by UPS.  Delivery fees are based on weight and distance.

Telephone:
You may also order forms by calling the SC Department of Revenue at 803.898.5320 or 1.800.768.3676 and leaving a voice message containing the following information:

• Name and mailing address where you would like the form(s) to be sent (please spell out name and address)
• Form ID number and title of form
