How To Get Withholding Forms

You may obtain withholding forms using one of four methods:

1. Visit the South Carolina Department of Revenue website at www.sctax.org.
2. Receive forms by fax by calling South Carolina Department of Revenue Forms Request System statewide at (800) 768-3676 or (803) 898-5320 in Columbia.
3. You may visit any of the six taxpayer service centers around the state.
4. You may call the Contact Center at (803) 898-5752.
