PREREQUISITE:
MUST HAVE A VALID, IN GOOD STANDING LIQUOR BY THE DRINK LICENSE – PLB and/or PLC

o A COMPLETED AND SIGNED ABL – 29 or ABL – 29A
Fees: Individual Sundays - $200 per Sunday (ABL-29 form)
52 Weeks (One Year) - $3,050.00 (ABL-29A form)
o COPY OF CURRENT BUSINESS LICENSE ISSUED BY THE TOWN OF MONCKS CORNER
o Immigration Documents (ABL-577 – for sole proprietors or an ABL-920 – other than sole proprietor) on EACH principal.
Principal Types:
· The owner (if sole proprietorship)
· All officers of the business or entity, which owns the business
· All partners
· All persons who own 25% or more of the value of the business entity
· All persons who own 25% or more of the combined voting power of the business or entity
· Managers of LLC, which is managed, by managers
· Members of LLC, which is not managed by managers
· Any fiduciary who manages, controls title, or is otherwise in control of the business
· All employees who will have day-to-day operational management responsibility for the business or entity
· If a publicly traded corporation, the designated agent (over 21 years of age and SC resident)
· All other principals must be listed also. If not publicly traded, all stockholders
· If a nonprofit organization, all officers and directors of the organization

MUST HAVE A VALID, IN GOOD STANDING ON/OFF PREMISE BEER AND WINE PERMIT – PBG or PBW
MUST SUBMIT NOW: MOST Necessary Forms Available Online – www.sctax.org
o A COMPLETED, SIGNED, AND NOTARIZED ABL-901
Including the Consent and Waiver.
Not Required: Newspaper receipt, Criminal History and Lease
o ORIGINAL GREEN LICENSE (COPY IS UNACCEPTABLE)
o SIGNED ACKNOWLEDGEMENT 7-DAY OFF-PREMISE BEER AND WINE PERMIT (For PBW’s Converting to P7B’s ONLY)
o COPY OF CURRENT BUSINESS LICENSE ISSUED BY THE TOWN OF MONCKS CORNER
o FEES: $1,200.00 (if application is received on or before 02/28/2010)
